IMPORTANT NOTICE OF KPS PLAN CLOSURE/CHANGES DECEMBER 1, 2011


We have some important information we need to share with you concerning your KPS Health Plans' Individual and Family plan clients.

Benefit Enhancements
As of January 1, 2012, you will begin receiving enhanced benefits from the removal of annual limits on some of their benefits. More information detailing these benefit enhancements will be provided to existing clients through a separate Contract Endorsement mailing which will be arriving shortly. A separate Notification will be sent with links to these materials in advance of the Contract Endorsement mailing.

Plan Closure
In addition to these benefit changes, KPS Health Plans will no longer be accepting changes to or new enrollment on its Centric Enhanced, Centric 30/2000, Centric 30/3000, Centric 30/5000, The Healthy Investor(tm) - Investor 2000/4000, and The Healthy Investor(tm) - Investor 3000/6000 plans beyond December 1, 2011.

However, coverage will continue into 2012 on existing policies. KPS will simply be closing these plans to new enrollment.  Please note that plan changes to higher or lower deductibles, within the KPS Individual plans, is considered new enrollment onto those plans, and will not be available for effective dates beyond December 1, 2011.

For those who wish to change plans, from any of our Centric Enhanced, Centric 30/2000, Centric 30/3000, Centric 30/5000, The Healthy Investor(tm) - Investor 2000/4000, The Healthy Investor(tm) - Investor 3000/6000 plans, timing is crucial.

Due to the plan closures, current members' plan change requests must be received no later than November 30, 2011 for a December 1 effective date.
